27. An applicant for the Haz-Mat endorsement must be one of the following:
- A citizen of the U.S. who has not renounced his/her U.S. citizenship
- A lawful permanent resident of the U.S. as defined in Section 101(a)(20) of the Immigration and Nationality Act
- A lawful nonimmigrant in possession of valid, unrestricted employment authorization
- A refugee admitted under Section 8 U.S.C. 1157 in possession of valid unrestricted employment authorization
- In asylum status under Section U.S.C. 1158 in possession of valid, unrestricted employment authorization

- I have not been convicted, or found guilty by reason of insanity, of any disqualifying crime listed in Section 1, Part A, in any jurisdiction, military, or civilian, during the 7 years before the date of this application.
- I have not been released from incarceration in any jurisdiction, military or civilian, for committing any disqualifying crime listed in Section 1, Part A, during the last five years before the date of this application.
- I have not been convicted, or found guilty by reason of insanity, of any permanently disqualifying crime listed in Section 1, Part B, in any jurisdiction, military or civilian.
- I am not wanted or under indictment for any disqualifying crime listed in Section 1, Part C.
- I have not been adjudicated as lacking mental capacity or involuntary committed to a mental institution.
