Haz-Mat Endorsement

As of January 31, 2005, all first-time CDL applicants wishing to obtain the Hazardous Materials endorsement will be required to undergo a Transportation Safety Administration (TSA) background check and fingerprinting prior to receiving a Haz-Mat endorsement. Central Tech continues to encourage students to obtain the Hazardous Materials endorsement because we feel that it will increase employment opportunities. It is recommended that all Oklahoma residents wishing to obtain the Hazardous Materials endorsement and planning to attend Central Tech Truck Driver Training complete the following steps before the first day of class:

1.Go to www.hazprints.com and complete the information requested, obtain a confirmation number, and pay $89.25 with a credit card (VISA or MasterCard). If you do not pay with credit card, a money order made payable to Integrated Biometric Technology will be required. To view a list of the types of questions asked, click here.

2.Go to one of the following fingerprinting collection sites. There are currently two locations open in Oklahoma:

  2. Certificate of Naturalization (INS Form N-550 or N-570)
  3. Unexpired foreign passport with I-551 stamp or attached INS Form I-94 indicating unexpired employment authorization.
  4. Driver's license or ID card issued by a state provided it contains a photograph or information such as name, address, date of birth, gender, height, and eye color.
  5. U.S. Military/Retire ID Card
  6. Military dependent's ID card
  7. ID card issued by federal, state, or local government agencies or entities, provided it contains a photograph or information such as name, address, date of birth, gender height, and eye color.
  8. Certificate of U.S. Citizenship (INS Form N-560 or N-561
  9. Permanent Resident Card or Alien Registration Receipt Card with photograph (INS Form I-151 or I-551)
  1. Voter's registration card
  2. U.S. Coast Guard Merchant Marine Card
  3. U.S. social security card issued by the Social Security Administration (other than a card stating it is not valid for employment)
  4. Original or certified copy of a birth certificate issued by a state, county, municipal authority or outlying possession of the United States bearing an official seal
  5. US Citizen ID Card
  6. Certificate of Birth Abroad issued by the Department of State (Form FS-545 or Form DS-1350) 7.Native American tribal document

Once TSA completes the security threat assessment, the applicant will be notified of the results by US Mail. Oklahoma Department of Public Safety will be notified whether the applicant may obtain or renew a hazardous materials endorsement.
